Application for Treatment
Tendonology adopts a strict but straightforward policy when accepting a horse for treatment. We do this to safeguard the security and well-being of the horse and also to ensure that we can provide the best possible care, treatment and service.
The following five documents should be read by all prospective clients/connections.
The documents highlight in red with a * must be downloaded, completed and forwarded prior to the horse’s arrival. They may in exceptional circumstances accompany a horse following a prior consultation with David Chapman-Jones. Tendonology cannot under any circumstance accept a horse for treatment without a signed treatment agreement form (document 5) and a deposit payment.

This form MUST be completed in full prior to the horse arriving. It provides us with all the information we need to know about the horse
The purpose of this document is to provide consent, given by an authorized person in relation to the care and treatment of the horse. It provides permission for the temporary transfer of care from the current authorized person to Tendonology.
